7th Grade Immunization Requirement

Parents of 6th graders… Please remember that Florida State Law requires your child to receive a TDAP immunization prior to entering 7th grade. If your child already received this immunization, please bring in their immunization record.  Now is a great time to make an appointment with the pediatrician before the summer begins.  You can also go to the local health department and receive the immunization for free.  See location below.

STUDENTS ENTERING A PASCO COUNTY SCHOOL FOR THE FIRST TIME AT ANY GRADE LEVEL MUST PRESENT THE FOLLOWING:

  1. IMMUNIZATION (SHOT) RECORDS MANDATORY:
    1. 6TH, 7TH, 8TH DPT, POLIO, MMR (MEASLES, MUMPS, RUBELLA) & HEPATITIS B SERIES; 6TH & 7th Varicella  (Chicken Pox Vaccine)
    2. 7TH & 8TH DTaP/Tdap (Diphtheria-Tetanus-Pertussis) WITHIN 5 YRS or TD (Tetanus-Diphtheria Booster) within the last two years.
  2. CURRENT PHYSICAL:  Dated and signed by a physician within the last 12 months.
  3. BIRTH CERTIFICATE:  or Custody/Guardianship/Adoption Papers (If applicable)
  4. SOCIAL SECURITY CARD
  5. REQUIRED 3 PROOFS OF RESIDENCY IN (CSRMS) ZONE:
    • Lease or Mortgage, signed Sales Contract, Tax Bill (pascopa.com)
    • Utility Bill connected to address; power, water, cable
    • Drivers License, Auto Registration, or Voter Registration (pascovotes.com)
  1. WITHDRAWAL FORM OR RECENT REPORT CARD FROM LAST SCHOOL ATTENDED
    1. If entering during the year, a WITHDRAWAL FORM from the previous school is required.)
  2. SPECIAL EDUCATION RECORDS
    1. (if applicable) – to include copies of current IEP, EP, SOCIAL HISTORY and PSYCHOLOGICAL report.
    2. LEGAL GUARDIANSHIP RECORDS (IF APPLICABLE)

 

STUDENTS TRANSFERRING FROM ANOTHER FLORIDA COUNTY MUST PRESENT THE FOLLOWING ITEMS :

  1. CURRENT IMMUNIZATIONS, PHYSICAL AND SOCIAL SECURITY CARD
  2. REQUIRED 3 PROOFS OF RESIDENCY
    • Lease or Mortgage, signed Sales Contract, Tax Bill (pascopa.com)
    • Utility Bill connected to address; power, water, cable
    • Drivers License, Auto Registration, or Voter Registration (pascovotes.com)
  1. COPY OF STUDENT’S LAST REPORT CARD (IF APPLICABLE)

STUDENTS ENTERING CSRMS FROM OUT OF STATE ALSO NEED A CURRENT PHYSICAL AS WELL AS THE IN COUNTY TRANSFER ITEMS LISTED ABOVE 


 

REGISTRATION PROCEDURES/POLICIES

  1. You must have all required documents, and necessary paperwork must be completed in order to register.
  2. Students may not register themselves.  Everyone must be accompanied by a parent/guardian regardless of age.
  3. Students will meet with a counselor to determine their schedule, which is based on the documents provided. Changes may be required after official records are received from the previous school and reviewed.
  4. Students living with someone other than their parents or legal guardian must provide a notarized statement from their parents/guardians stating that those person(s) are legally responsible for the student.
  5. If you do not live in the Rushe Middle School zone, a School Choice Application form must be completed and submitted to the School Board for approval.  You will be notified by mail of their decision.

Middle School Course Requirements

According to the State of Florida rules, middle school students must successfully complete 3 years of Language Arts, 3 years of Mathematics, 3 years of Science, 2 years of Social Studies, and 1 year of Civics. Successful completion of these courses requires the earning of a grade of “D” or higher.

In addition, a one-semester course, which includes Career and Education Planning, must be completed by the end of the 8th-grade year.  This course includes using the online student planning & advising system known as My CareerShines, which results in the completion of a personalized academic and career plan.

Students will not be promoted unless the academic history is complete.

If your child is from out of state, the student will still need to meet Florida requirements.
