The Florida Department of Health requires all incoming 7th graders to have the following immunization:
- Tdap (1 dose) Tetanus-Diphtheria-Pertussis
Documentation of the Tdap vaccine is required BEFORE your child starts 7th grade (must be within 5 years). Your child will not be able to attend classes until this information has been provided to the school (on or before registration day in August). Your child will not receive their schedule at registration unless we have a record of Tdap immunization. Current 6th graders should already have received the following immunizations:
- MMR (2 doses)
- Polio (4 doses)
- DTP/DTaP (5 doses)
- Hepatitis B Series
- Varicella vaccine (2 dose) or date of disease
You may want to take your child’s complete immunization record with you when you go to obtain the vaccine. Return the completed DH 680, Florida Certification of Immunization, to the school after your visit, so this information can be added to the electronic database. Immunizations are available from your private physician or local health department at the following locations:

Vaccinations are free for children up to the age of 18. No appointment is needed.
